Print and fill out this reservation form then fax it to our office at 1-866-681-5685. Once we have received your form, we will fax you a proposal detailing the total cost of your request.

To complete and confirm the reservation, sign and fax the proposal back to us and we shall be present in your class on the reserved date.

If you must leave a voicemail, mention as many details as possible : your name, name of the school, its phone and fax numbers, type of presentation you’re interested in, number of presentations, number of students per presentation, schedule, and choice of dates (2 if possible).

Once we have all the pertinent information, we will fax you a proposal detailing the total cost of your request.
